Problem Statement – Amina Johnson iHuman case study
This 23-year-old female with a history of mononucleosis presents with acute pharyngitis and associated fever, headache, and tender cervical lymphadenopathy. The physical exam is notable for a temperature of 101.5° F orally, posterior pharyngeal erythema, mild to moderate tonsillar enlargement without uvular deviation, prominent tonsillar exudate, and tender bilateral anterior cervical lymphadenopathy without nuchal rigidity or hepatosplenomegaly.
Management Plan – Amina Johnson iHuman case study
- Give amoxicillin 500 mg PO tablets BID x 10 days
- Give ibuprofen 200 mg PO tablets, 2 tablets every 6 hours, do not exceed 8 tablets in a 24-hour period
- Teaching note. If cuiture not already ordered, obtain throat culture
Supportive Care
- Rest.
- Maintain adequate fluid intake
- Gargle with saltwater.
- Observe a soft diet and add cold food items such as popsicles or slushies
- Avoid irritants such as spicy foods or second-hand smoke.
Patient Education
- Educated patient on the high likely diagnosis of strep throat, current test results, empiric treatment regimen, and prognosis
- Educated patient on medications, including indications, proper administration, side effects, and red flag symptoms for discontinuation. Advised the patient to finish all of the antibiotics as prescribed, even if they start to feel better.
- Advised patient to replace toothbrush after being on antibiotics for 48 hours to avoid re-inoculation.
- Discussed the use of a secondary form of birth control while on the antibiotic.
- Recommended patient isolate at home and avoid contact with others until on antibiotic for 24 hours
Follow-Up
- Follow up in the clinic if symptoms do not start to resolve within 48-72 hours.
- Discussed warning signs with which to call the clinic immediately or to go to the ER, including but not limited to, inability to swallow, throat swelling, difficulty breathing, inability to maintain adequate fluid intake, severe increase in pain, inability to open the mouth, or for other concerning symptoms Amina Johnson iHuman case study
