CASE STUDIES: PROFESSIONAL ROLE DEVELOPMENT

“Nursing research involves much planning and attention to details, yet novice and seasoned nurse researchers often overlook the day-to-day operations required to conduct research studies. Project management is a set of iterative steps that can facilitate the process of conducting nursing research” (Rew et al., 2020).

As you begin working on your small nursing informatics project, consider how this project will plant the seed for your future work (in and out of an educational setting). How might what you do in this course lend itself to what you will need to do in nursing practice on a day-to-day basis? Why might it be important to consider this now versus later in your education or practice?

Throughout this course, you are tasked with implementing, or proposing an implementation, of a small nursing informatics project. This project is steeped in project management concepts to drive your work.

For this Discussion, you will  explore why this approach might not only will assist you in this course, but how it might assist you in nursing practice beyond your studies.

TO PREPARE

  • Review the Learning Resources for this week and reflect on the roles of advanced nursing practice as it relates to project management.
  • Select one of the roles described in Chapter 8 of the Sipes text to focus on for this Discussion.
  • Review the article by Rew et al. (2020) and reflect on the steps and processes used by the team described in the article.
  • How might you use a similar approach for the completion of your DNP doctoral project or dissertation? What project management strategies might be most appropriate for your nursing research?

BY DAY 3 OF WEEK 2

Post a brief description of 4–5 major competencies required for the advanced nursing practice role that you selected.

  • Explain how the project management concepts you have examined thus far in the course might align with this advanced nursing practice role and why. Be specific.
  • Based on the steps and processes used by Rew et al. (2020) and the resources on techniques and tools for nursing research, how might you use these same concepts and a similar approach for completing your DNP doctoral project or dissertation?
  • Explain which processes and techniques you believe will provide the most guidance to assist you plan and develop your DNP doctoral project or dissertation. Be specific and provide examples.   • The above-mentioned process and technique will provide the most guidance in planning and developing a DNP doctoral project. Furthermore, the following steps would be included: Creating a project scope, breaking it down into task/work packages, and creating a schedule/timeline Without a plan to provide structure and direction, the project can become disorganized, jeopardizing project constraints such as budgets, schedules/deadlines, resources, and other factors (Sipes, 2020). Understanding the project scope, setting clear expectations, using a work breakdown structure, and documenting the process are project management strategies that I will learn as a nurse leader and apply to facilitate nursing research in my DNP doctoral project. To complete a successful research project, the Centers for Disease Control and Prevention encourages, researchers to use project management principles (Rew et al, 2020). Project management principles are learned through practice and repetition (Sipes, 2020). Finding the gap and creating a measurable, obtainable, and realistic project management includes researching the current guidelines, evidence, and research to support the proposed project and deliver it to all key stakeholders (Sipes, 2020). Having a clear purpose, following an algorithm, and a well-designed study, enable us to provide Specific, Achievable, Measurable, Realistic, and Timed (SMART) outcomes (Howarth, Probyn, & Maz, 2017).
  • The project management concepts may vary in different phases.  These competencies include assessing or identifying the problem, designing, or initiating projects, data collection, analyzing, delivering these findings to the key stakeholders, implementing or executing the plan, monitoring or controlling the project, and closing or completing the task (Sipes, 2020). The nursing research process included the following phases: phase 1: conceptual, phase 2: design and planning, phase 3: empirical, phase 4: analytic, and phase 5: dissemination (Rew, Cauvin, Cengiz, Pretorius, & Johnson, 2020). The conceptual phase is when the principal investigator and research team identify the problem, review relevant literature, choose a conceptual or theoretical framework, and propose a research design to answer research questions or test research hypotheses” (Rew et al, 2020). During the design and planning phase, the research design, appropriate measures for theoretical variables, and the appropriate population were chosen, the intervention protocol was planned, a sampling plan, a plan for human subject protection, and a budget were developed (Rew et al, 2020). The empirical phase of nursing research is where data from study participants is collected and prepared for analysis (Rew et al, 2020). The data is analyzed, and the findings are interpreted during the analytic phase (Rew et al, 2020). The dissemination phase is where the study findings are shared with other researchers, clinicians, data collection facilities, and the funding agency (Rew et al, 2020).
  • Clinical knowledge and skills, financial management skills, leadership skills, interpersonal skills, and communication skills are all required for advanced practice nursing roles. Some of these skills, such as interpersonal, communication, and leadership aptitudes, promote collaboration, which is essential in the clinical setting (Busari et al., 2017). Furthermore, advanced practice roles necessitate that nurses be proficient in the use of technology.

    •             The role you chose is very interesting I love learning about new roles. This is a role I feel that would be extremely focused on how to establish roles and change roles so that it is most effective. Knowing what data needs to be collected as well as what technology needs to be used it. Dealing with project management in different phases would be difficult and always changing this is where a future plan would come in as well as being organized with your information.     • I enjoyed reading your post, Vicky Awken Ngang.  It is true that advanced practice nurses (APRNs) must possess certain characteristics and/or competencies given their level of education.  As a terminal degree, the doctoral-prepared nurse must have competence in managing healthcare finance using cost-effective approaches to minimize healthcare expenditures.  As an example, APRNs at the doctoral level should order medications that are cost-effective and order inexpensive tests and procedures. Considering the level of education and knowledge they possess; they must possess the ability to lead.   The importance of interpersonal skills in health care settings and at the doctoral level cannot be overstated.  To collaborate effectively with interdisciplinary teams, APRNs need interpersonal skills. Having the ability to relate to colleagues, patients, and relatives in a humane and person-centered manner is considered an interpersonal skill (Reyes & Alvarez 2022). To provide effective, safe, and timely care, APRNs must be proficient in technology.  It is imperative to be computer literate, especially now that telemedicine and telehealth are at the forefront of healthcare.  It is essential to possess technology proficiency since it increases access to care, utilization, and treatment.   As a nurse, I agree that the first step in project management is to identify the healthcare problem that is affecting the patient’s health.  Identifying the problem is followed by planning, designing, and collecting the data.  First, identifying what is happening, planning, designing, collecting data to test the hypothesis, building a team, monitoring the project, and setting timelines are all important steps that project management entails, which are aligned with the nursing process as well as help us develop and complete our doctoral dissertation. Also involved with the completion of dissertations are meeting deadlines, teamwork, risk control, and project assessment to identify potential risks and prevent them from becoming a problem and setting a deadline. Just to add to your discussion, advanced practice registered nurses (APRNs) who obtain project management knowledge and skills are more likely to be successful in nursing practices and policies (Peterson et al. 2015). Project management is a broad and encompassed process in which a research study is carried out systematically to accomplish a final task. Applying project management skills are critical in healthcare for quality patient outcomes. There is no doubt that nursing informatics is fast evolving; hence adequate knowledge of informatics supports nurses, consumers, patients, the interprofessional healthcare team, and other stakeholders in their decision-making in all roles and settings to achieve the desired outcome (Sipes, 2016). Nurse leaders must understand the basic framework which guides project management steps toward achieving an informatics study, and these steps include-Design/Initiation, Plan, Implementation, Monitor/Control, and Evaluation/Lessons Learned. The goal of nurse leaders skilled in informatics is to improve the health of populations, communities, families, and individuals by optimizing information management and communications (Williams, 2019).   •             The processes I feel are most important are learning how to effectively analyzing data and organize data in a way it can be easily utilized. Collecting the data is not difficult ensuring you are correctly applying and utilizing the data is the difficult part. By doing this while putting together my DNP project it will help be understand my process and apply my theories to practice. As a nurse leader I will be able to create change in a way that will be effective and be able to sustain which is my main goal. DNP is a powerful strategy to prepare nurses for the most advanced clinical nursing practice level, i.e. “providing leadership for evidence-based practice”. (Edwardson SR, 2010)

The competencies required in this area are as follows: leadership, organization, analyzing data, and collaboration.     • I totally agree with you that some of the major competencies required for Nurse Practitioners (NP) include clinical and professional leadership competencies and many more such as knowledge abilities, communication, interpersonal skills, data analysis, and utilization to improve healthcare delivery and leadership skills. Nurse Practitioners apply quality care by utilizing evidence-based practice to improve the quality of clinical practice and the ability to use technology to organize effectively, be detail-oriented, and function as an independent practitioner are all core competencies expected of a nurse practitioner (Thomas et al., 2011). In addition, the concepts, that guide the project management competencies are essential to Doctoral prepared nurses.  Using the SMART objective to plan projects as specific, measurable, achievable, relevant, and timely and equally important tools in the process (Sipes, 2020).

    • As a professional and clinical leader, there are many responsibilities one has to accomplish. For one to be a great leader in the clinical and professional area the following are attributes one requires collaboration, analyzing data, organization, and leadership. The concepts learned in DNP are critical for one to become a clinical and professional leader (Edward). The concepts include project management, critical thinking, time management, and among others.
      Leadership is inborn but it can be natured and manifested in many ways. One can be a leader clinically or professionally using concepts learned in the DNP course. The steps and processes will assist one to communicate effectively and collect data to lead the company effectively. The medical world is dynamic, and systems are changing gradually (AACN).
      Landscapes in healthcare are becoming more complex and it calls for nurses or supporting professionals to improve their hard work. Leadership nursing requires resilience and hard work for one to perform tasks effectively. The skills utilized in leadership can assist one in developing a strong DNP project and dissertation. DNP is powerful enough to prepare an individual for any task in the nursing field.

Population health works on collaborative approaches to disease management and heath prevention amongst multiple healthcare facility settings to improve the outcomes of populations.  Quality and safety are based on scientific improvement to decrease risk of harm to patients and providers alike, through enhancing the quality of care in systems and performances. Interprofessional partnerships is a collaborative method with different members of the care team, patients, family, the community, and other stakeholders to improve healthcare outcomes and experiences. System based practice is when nurses provide safe, equitable, quality care to diverse populations through providing resources.            According to Rew et al, (2020), Project management is a set of Iterative steps that can facilitate the process of conducting nursing research. The concepts I have explored align with my practice and my project by assessing the gaps in patient needs and developing improvements for practice. For example, my project is looking at HCAHPS scores, and how patients are affected by these numbers, Particularly the low scores in Understanding the purpose of taking medications and information regarding symptoms to look for.  I am looking at a method to improve patient comfort with their medications as well as signs and symptoms to look for once they are discharged to decrease poor outcomes and improve their confidence with independent medication administration at home. There is a major gap in this education, and it needs to be addressed and improved, Sipes, (2020).The ability to work directly with this facility on research projects prior to starting my doctorate project has been helpful. Now that I can apply classroom concepts to real word settings both prior to this project and currently as I begin it, I feel more confident in doing so.Explain which processes and techniques you believe will provide the most guidance to assist you plan and develop your DNP doctoral project or dissertation.   •             The advanced nursing practice role I’ve selected is the Psychiatric Mental Health Nurse Practitioner or PMHNP. According to Kumar et al. (2020), major competencies that are required for this role are data informed planning, established relationships with other entities, use of evidence-based guidelines, family and youth focus with coordination, clinical expertise, communication, understanding of and implementation of the code of ethics, and leadership skills. These competencies are important for the APRN specializing in PMHNP as they will assist the APRN in providing the highest level of care to the patient and to both manage and lead a multidisciplinary team to ensure the best possible patient experience and outcomes.            The same concepts can be utilized in the process of completing the DNP Project or dissertation. Rew et al. (2020) defines processes or steps that are important to Nursing Intervention Research which may also aid in the completion of the DNP Project or dissertation. The five phases that Rew et al. (2020) mentions are phase one: conceptual, phase two: design and planning, phase three: empirical, phase four: analytic, and phase five: dissemination. In addition, I think PMHNPs possess core competencies in direct patient care involving resolving issues in the current operations and implementing policies in order to enhance patient care while fostering a positive relationship with the stakeholders, collaboration, staff management and maintenance of department budget (Sipes, 2020). It is true that the concepts of project management provide a clear guide for the steps of conducting research including the DNP project. I believe that it is important for nurses who participate in project management to stay active in the process and continue to remain involved in projects within their organization. It is important for advanced nurses to embrace the concept of innovation, innovation adaptation refers to the implementation of creative ideas, new technologies, and knowledge with proper strategic planning to enhance productivity and increase output with similar input (Tucmeanu et al., 2022). This is another big thing that I think we all need to incorporate into our professional practice. The nursing profession and healthcare are always changing and always evolving, and it is vital for us to be thinking outside the box and be openminded to ideas that can improve our profession and nursing care. The advance practice nurse also needs to have good leadership skills. Project management requires higher level leadership to ensure successful completion of a project. For effective completion of the DNP, similar concepts like planning, team building, time management, risk control, and setting a deadline must be incorporated. The first phase of the nursing research process is generally referred to as the conceptual phase in which the principal investigator (PI) and research team identify the problem, review the appropriate literature, select a conceptual or theoretical framework, and propose a research design to answer research questions or test research hypotheses (Rew et al. (2020).  A research project’s first phase is the planning phase, according to the authors, when the investigator identifies the problem, conducts a comprehensive literature review to gather adequate information and evidence for the problem under study, and creates a framework, all of which the author elaborated on and that I should incorporate in my DNP project. The execution or control phase would then follow where the goal is to keep everyone informed and strictly follow the planned timeframe. The last step after completing all activities is a comprehensive evaluation of the results I would achieve from my project.             These assessment procedures and methods will give me the most significant direction as I plan and create my DNP doctorate dissertation. I could improve my professional advocacy, research, and accountability with the support of the core skills.   • Initial Post            The role selected for this post is Chief Nursing Officer (CNO). The CNO is responsible for the daily operations of the healthcare center, clinical operations, budgets, and staffing. The competencies required for this role are knowledge of the healthcare environment, leadership, financial literacy, communication skills, and relationship building. The CNO is responsible for setting patient care standards, developing processes and protocols, setting budgets for operations, and mentoring/coaching/teaching the clinical staff. The competencies listed above shape the operations of a healthcare facility. The CNO sets the standards of operations for the clinical staff.           Project management is an essential component of process improvement. “Managing a project includes identifying requirements, establishing clear and achievable objectives, balancing competing demands of quality, scope, cost, and time, adapting specifications, plans, and approach to meet expectations of all key stakeholders including the client and the end-user” (Anantatmula, 2020b). Taking this into consideration, I would use the competencies to narrow the focus of my project. As discussed in Rew, 2020 during the conceptualization phase the research should identify the problem, review the appropriate literature, select a conceptual or theoretical framework, and propose a research design to answer research questions or test research hypotheses. I would also develop a sampling plan, determine how data will be collected, and develop the measurements to ascertain effectiveness.           The process I feel will provide the most guidance to my project is the sampling plan. Gray and Grove (2020, pg 410) state sampling involve selecting a group of people, events, behaviors, or other elements with which to conduct a study. A sampling plan defines the process of making the sample selections; the sample denotes the selected group of people or elements included in a study. Sampling decisions have a major impact on the meaning and generalizability of the finding. If the sample size is too small or subjects leave the study before the conclusion of the study, it may not be valid. It is such an excellent discipline to choose from. Besides the competencies you selected above to expedite the role of the chief nursing officer, an evaluation tool must be developed at the beginning to establish their leadership skills (Ulrich et al., 2019). The assessment tool is used to test competencies such as communication and management of the workers. Project management skills will require research from the existing literature on the existing samples to check if the set strategies may encounter hiccups in the future. To develop a project plan, one has to check on the existing project plans in the available literature and correlate them with the plan’s objectives. The sampling plan will work best for your project because a good plan will be developed after establishing a comparison of the existing plans on the latest literature reviews (Crisp, 2018).

Like advanced nursing practice, project management concepts are needed in designing care plans.            In advanced nursing, some critical competencies include financial management, interpersonal skills, clinical knowledge, and communication skills. Successful completion of nursing informatics projects requires project management knowledge like timetables, budgets, and objectives. Advanced nursing practice plays a significant role in the planning and implementation of nursing informatics projects. Additionally, advanced nursing practice ensures projects are successfully completed and satisfy the requirements of patients and nursing staff.   According to Sipes (2020), the advanced nursing practice provides comprehensive examples of the necessary tools used in projects and helps to explain how these tools can be applied to complicated healthcare tasks. Like advanced nursing practice, project management concepts are needed in designing care plans. The project’s goals need to be accomplished to ensure patients and medical staffs are satisfied with the final products.            DNP dissertation or doctorate project can be completed as explained by Rew et al., (2020) article. This involves following all the techniques and procedures. Besides following doctorate project timelines and objectives, developing a budget is also critical. To complete my doctorate project, as an advanced nurse I will follow all processes described in the article. For example, I will apply the empirical phase to collect data from the participants. After collecting data, I will use tools for example SPSS to analyze both qualitative and quantitative data. The tool will help me to evaluate data for accuracy and determine statistical assumptions. Effective completion of a DNP doctorate dissertation or project means following the necessary procedures like properly utilizing the proposed budget (Rew et al., 2020). The ability to complete a DNP project on time depends on the proper management of goals. In this case, I will use the Project Management Body of Knowledge (PMBOK) technique to finalize the research process, like archiving related documents to be used in historical data.In a DNP doctorate project, planning and development of the research topic depend on the timeframe, budget, and objective. Following these procedures will ensure the project is completed on time without necessarily exceeding the allocated costs. Notably, it is important to apply project management skills to support nursing practice in a DNP doctorate project. Some of these tactics involve following deadlines, budgeting, and setting clear objectives. According to Howarth et al., (2017), projects fail because of poor planning. The project will have clear goals, explained objectives, and timescales to prevent this problem. Additionally, it is essential to use project milestones in the DNP doctorate project to provide guidelines on the final stage of the project. Some of the crucial points in the project include ethical approval, accessing the sample population, funding the research, and analyzing the research activities.            Advanced nursing practice plays a significant role in the planning and implementation of nursing informatics projects. Depending on their organization and where they work their role can look different in the project management process. Project planning and management have a significant impact on the goals and workload of the nursing profession, project teams in healthcare are often interprofessional and interdisciplinary in composition and charged with macrosystem organization-wide initiatives (Smith & Johnson, 2019). Depending on what role the advanced practice nurse has taken on you have listed important competencies that they must have. Financial management, interpersonal skills, clinical knowledge, and communication skills are very much needed especially if in a leadership role. Project managers must have the appropriate skills and competencies to ensure that the project is successful. Often project managers have limited research management training, are consumed with coordinating more than one project, or have other job responsibilities and commitments that can distract their focus; when projects are poorly managed, errors can occur such as protocol deviations, inconsistencies in data collection, and missing data (Aycock, et al., 2017). With this in mind, it is vital that we take the time to develop the skills necessary to ensure that projects that we work on such as our DNP projects are successful.

    • Ijeoma, thanks for your post. As healthcare in the United States continues to evolve under mounting cost and quality pressures, the need for project management cannot be underestimated. Project management in healthcare settings positively impacts projects focused on improving healthcare-related organizations’ operations. Healthcare organizations that practice project management will likely benefit from reduced costs and better performance (Shirley, 2020). The benefits will probably be more profound when the nursing workforce in advanced nursing practice roles is knowledgeable in project management. For example, when advanced practice registered nurses (APRNs) have project management knowledge and skills, they are likely to be more successful in improving nursing practice and in quality initiatives and implementation. I wish to add to your post by noting that the project management concepts discussed align with the roles of APRNs, mainly because they are frequently involved in numerous healthcare projects. When they understand the various project management concepts, APRNs can gain unique knowledge and skills that would enable them to succeed in project management-related roles such as budgeting, scheduling, and resources management (Sipes, 2016). Besides, they can help their healthcare organizations maintain a clinical practice, conduct program evaluations, implement practice changes and improvements, manage quality, translate research evidence into practice, improve healthcare delivery, improve productivity, reduce costs and workload, and reduce risks. The four significant competencies for this role are informatics, project management, nursing research, collaboration, and education.  Sipes (2020) notes that Project management is a foundational concept in informatics. However, nurses have used the skills under this umbrella today in various ways for several years. For example, having the ability to log on and document in a computer where the computer is the primary mode of entering patient data has been a function nurses have assumed for several years.   The preceding phases of nursing research might align with the DNP doctoral project by providing a guideline for the project, specifically by identifying a problem reviewing the appropriate literature, selecting a research design, developing questions, planning interventions selecting the population, measuring variables collecting and analyzing data and drawing conclusions and communicating findings all within a given time frame.

    • Thank you for sharing your post with us Lizzie. The role I selected is the DNP prepared nurse with a specialization in psychiatric mental health nurse practitioner or PMHNP. An article was written by Kumar et al. (2020) which mentions major competencies that are important for this role. The major competencies important for this role are data informed planning, established relationships with other entities, use of evidence-based guidelines, family and youth focus with coordination, clinical expertise, communication, understanding of and implementation of the code of ethics, and leadership skill (Kumar et al., 2020). These are important competencies for the DNP prepared nurse specializing in PMHNP because they will assist the DNP prepared nurse in providing the highest level of care to the patient as well as managing and collaborating with a multidisciplinary team. Other concepts that are important for the DNP prepared nurse specializing in PMHNP are having well-defined project goals/ deadlines and objectives, having clear team roles and responsibilities, being transparent, establishing means of accountability and responsibility, utilizing evidence-based practice, and utilizing effective and efficient communication skills (Sipes, 2020).   • The entire process is useful for the DNP project. The conceptual phase is doing the research and figuring out what practice problem needs to be improved. The design and planning phase is creating the design of the project of trying to improve the practice problem. This is figuring out who the patient population is going to be how is data going to be collected? Addressing financial aspects of the project. Empirical phase is the actual collection and preparation of data. Analytic phase is the statistical analysis of all the data and interpretation of the results. The dissemination phase is the communication of the results of the project. Learning the project management skills will help me throughout the DNP project process because the focus of these skills is to learn and apply the research process. Understanding the process and why it is done the way it is done is important because it will help me to successfully complete my DNP project. It will give me an understanding before the project is started which will make it smoother for me during the project and have a deeper understanding of the results of the project and how to apply it once I have successfully completed the DNP project and begin applying it in my professional practice.
  • Rew at al. (2020) do an excellent job of describing the steps and processes that are used for project management. I think that the way it is broken down into different phases and each phase is broken down into more detailed tasks is a great guideline for me to utilize when completing my DNP project. Following the roadmap, they lay out can help successfully complete each step during the DNP project.
  • The 5 project management concepts are design/initiate project, develop project plan, implement project plan, monitor, and control project, and close project and evaluate lessons learned (Sipes, 2020). The concepts of project management mirror the concepts of the nursing process. We are taught to apply these concepts in nursing school which are assess, diagnose, outcomes plan, implement care plan and evaluate plan. These concepts are very similar, and the project management concepts align with the NP role because it is just a shift in focus for the NP. The NP is more direct patient care related but applies the same concept to patient care and patient plan of care. PM is the same process but focused on the improvement of whatever project is chosen. 
  • The role that I chose was a Nurse Practitioner and 4 major competencies required for a NP are information management, information literacy, computer skills, and the ability to work with datasets and databases (Galacio, 2022). It is important for a NP to have these competencies because project management requires the NP to be able to effectively use a computer and be able to navigate as needed to gather information or data that is needed. NP’s must also be able to not only gather information and data but they must also be able to interpret and understand the data that is being obtained. Knowledge of how to use the data is important as well because data that is gathered needs to be used effectively to assist with the end goal of the project.
