NR584NP Week 3 Assignment: Risk Management Case Study Template

NR584NP Week 3 Assignment: Risk Management Case Study Template

Dear Students:

In week 3, you must complete a worksheet analyzing a case study. For this session, here is the case study under consideration:

A healthcare risk management case emerges when a patient sues an outpatient provider due to a missed cancer diagnosis related to a failure to conduct timely and appropriate colon cancer screening. A patient visits an outpatient clinic complaining of gastrointestinal symptoms, but the provider overlooks the patient’s risk factors and fails to recommend or perform a colonoscopy as part of the screening protocol.

NR584NP Week 3 Assignment: Risk Management Case Study

 Name:

General Instructions:

  1. Review the Week 3 Risk Management Case Study scenario in the course’s Announcements section.
  2. Use this template for the assignment. Use of this template is required. If the template is not used, a 10% deduction will be applied. See the rubric. Save the template and include your name in the file name.
  3. Complete this using the provided scenario.
  4. Follow APA grammar, spelling, word usage, and punctuation rules consistent with formal, scholarly writing.
  5. Provide an in-text citation from three scholarly sources to support your writing.
  6. Abide by Chamberlain University’s academic integrity policy.

Complete the following sections:

  1. Identification
    1. Identify the risk.

The risk in the scenario entails a missed cancer diagnosis due to the failure of a provider to adhere to colon cancer screening guidelines, leading to delays in detection and treatment.

  1. Select the most appropriate classification of the risk.

The risk is categorized as a clinical care risk, as it emanates from inadequate adherence to evidence-based clinical protocols for the screening of colorectal cancer.

  1. Describe the risk domain.

The risk domain is diagnostic error, particularly related to delayed and missed diagnosis. The domain entails failures in timely and accurately recognition of a patient’s condition due to oversight of symptoms and the failure to implement any diagnostic measures.

  1. Contributing Factors
    1. Describe four factors that contribute to the risk.

There are four factors contributing to the risk as provided below:

Patient factors: The patient, in the case, presented with gastrointestinal symptoms and potential risk factors for colorectal cancer, which are either not recognized and not adequately considered during a visit.

Provider oversight: The healthcare personnel did not adhere to the screening guidelines for colorectal cancer and neglected to recommend and perform colonoscopy despite the existence of symptoms and risk factors.

Communication barriers: Ineffective communication on the essence of screenings and shared decision-making may have contributed to the failure of addressing the patient’s symptoms appropriately.

Systemic factors: Lack of adequate reminder systems and protocols for colorectal cancer screening in the workflow of a clinic thus resulting in missed opportunities to recommend colonoscopy.

  1. Proactive Measures
  2. Discuss three proactive measures to reduce or prevent the risk. Provide an in-text citation from one scholarly source to support your writing.

In preventing the risk or reducing it, there are proactive measures that can be taken as discussed below.

First, education and training would help as it would involve conducting of regular provider training sessions on current guidelines for colorectal cancer screening, while emphasizing the essence of evaluating risk factors and timely screening.

Second, clinical decision support systems (CDSS) should involve implementing of CDSS tools to alert providers on the requirements needed for screening based on risk factors of a patient while fostering adherence to evidence-based guidelines (Sutton et al., 2020).

Third, standardized screening protocols should be formed for evaluation of gastrointestinal symptom evaluation, while ensuring that patients meet the needed screening criteria and are referred to colonoscopy.

  1. Mitigation Strategies
    1. Describe three strategies the advanced practice nurse may use to mitigate the identified risk. Provide an in-text citation from one scholarly source to support your writing.
      1. Comprehensive patient assessment- It is imperative to conduct thorough evaluations to assess the risk factors, including family history and severity of symptoms to ensure appropriate measures for diagnosis are taken.
      2. Shared decision-making– there is a need to engage patients in discussions on the importance of colorectal cancer screening and work on decisions on timely colonoscopy referrals (Davidson et al., 2022).
      3. Follow-up systems- There is a need to have robust follow-up system to track patients with gastrointestinal complains to ensure timely diagnostic procedures that are scheduled and completed.
    2. Regulatory Foundations
      1. Identify at least one regulatory agency that provides guidance to manage risk. Provide an in-text citation from one scholarly source to support your writing.

The Centers for Medicare and Medicaid Services (CMS) provide guidance on the quality measures for the screening of colorectal cancer as part of quality payment programs (Blackstock et al., 2021).

  1. Discuss applicable regulations.

CMS guidelines adhere to colorectal cancer screening for eligible patients, with quality metrics tied to performance. Besides, the Agency for Healthcare Research and Quality (AHRQ) promotes best practices for diagnostic errors through the resources and patient safety initiatives.

  1. Reflection
    1. Describe how the advanced practice nurse can monitor the application and efficacy of the mitigation strategy.

The advanced practice nurse can use quality tools for improvement, such as periodic audits of screening adherence and outcome measures to evaluate the efficacy of protocols that have been implemented. Feedback acquired from patients and staff to further inform improvements.

  1. Discuss how you will apply the mitigation strategies in your practice.

I will use CDSS support to allow for guideline-based screening and place priority on patient education on the essence of measures of cancer prevention, routine evaluation of clinic workflows will help to foster protocols.

  1. Reflect on what you have learned from this assignment.

The assignment has fostered my understanding of the essence of proactive management of risks to lower the errors of diagnoses and improve outcomes for patients.
