According to the VA Office of Rural Health, almost 2.7 million veterans live in rural or highly rural communities, where even basic healthcare may be hundreds of miles away (VA Office of Rural Health [ORH], 2016).  In order to meet the unique need of these rural Veterans the VA initiated a robust initiative to increase access to health care.  In 2016 the most promising of these remote initiatives include telephone-based delivery of care, connecting veterans with veteran eligible resources, community incentives for healthcare training, and telehealth/telemedicine programs (ORH, 2016).  For this discussion I am going to focus on providing remote healthcare that eliminates the need for driving distances in light of the technological challenges to providing privacy and security protections. In using telehealth programs, the information exchange is not simply between the patient and the clinician, in that, many of these programs are maintained by a third party and this poses a security challenge for all concerned (Croll, 2010).  In fact, the concern for security breach is often mitigated by the trust the patient has in the clinician and the clinician’s unawareness (Croll, 2010).  Therefore, the ethical concern is the risk of security breach versus the health outcomes for the Veteran……

Traditional schools of medicine have a ritual of reciting oath excerpts such as the one above during their graduation ceremonies. Such excerpts usually revolve around a professional’s promise to uphold the ideals of patient safety and confidentiality to the best of his or her ability.

With the continued integration of HIT, and advances in technology such as hand-held computers, new ethical considerations have evolved within health care settings. For example, wireless capabilities can provide easier access to information from unauthorized outside parties. While technological advances have led to improvements in health care, they have also created new vulnerabilities. Even the most technologically adept organizations are susceptible to security breaches. The Department of Veterans Affairs (VA) is an industry leader, known for both its Telehealth system and electronic health records (EHR). As a government entity, the VA has publicly reported unintended security failures. To date, the VA has categorized the following breaches: lost and stolen laptops, employee errors in information sharing, e-mails to the wrong account holders, hackers breaking into databases, and even acts of malicious employees. Despite the VA and other health care organizations’ preventive measures, ethical and legal breaches still pose a very real danger to patients.

This week introduces the need for constant diligence when working with patient information. Through this lens, you examine the ethical codes and laws that govern today’s use of health information technologies.
